Background: IL18 (also known as IFN-gamma-inducing factor or IGIF) is an emerging pro-inflammatory cytokine with the capacity to enhance natural killer (NK) activity in spleen cells. It stimulates T cells to produce IFN-gamma and, when combined with IL12, demonstrates inhibitory effects on IL4-dependent IgE and IgG1 production, while promoting IgG2a production in B cells. Its activity can be modulated by IL18 binding protein (IL18BP), which interacts specifically with IL18, exerting a negative regulatory influence on its functions. Beyond these roles, IL18 can stimulate the expression of Fas ligand and induce the production of CC and CXC chemokines. Moreover, it has been found to increase the production of competent HIV. Structurally related to the IL1 family, IL18 engages with receptors including IL1 receptor-related protein, a member of the IL1R family. Similar to IL1, IL18 is initially synthesized as a leaderless precursor and requires the IL1 converting enzyme (ICE) for cleavage into its active form. Notably, IL18 might contribute to tissue injury during inflammatory reactions.
